GraceLyn Group provides a variety of habilitative services to support persons with disabilities in creating healthy, safe and fulfilled lives.

The following list of services are generally funded through formal or informal waiver programs which must be approved by the individual’s county social service agency.

| Supportive Living Services
Daily residential based habilitative services for individuals who require a 24 hour plan of care, supervision, support in activities of daily living and in the building of independence skills. Through The Relationship Approach, we assist individuals in living successfully in their community and inspire them to take action in being all they can be. Currently, we do this in a home specially designed to meet the needs of the individuals who live there. We invite you to check out Koinonia House, a licensed corporate foster care home for four young men in Apple Valley, MN.

| In-Home Family Support Services
Hourly habilitative services provided in an individual’s parental or caregiver’s home. Skilled staff assist individuals in moving forward in their day and teaching them relationship skills to enhance cooperation, contribution and self-discipline and independence skills for all areas of their life.

| Behavioral Strategist Services
Behavioral support and planning with an experienced professional to assist in creating relationship based strategies to interrupt behaviors and assist the person to move forward and create healthy interactions with themselves and others in a variety of settings.

| Supportive Employment Services
Vocational, volunteer, and/or recreational opportunities in the community for individuals who need an innovative and individualized way in which to learn work skills, find employment and enrich their lives.
